INTRODUCTION
L’Association canadienne-française de l’Ontario, Conseil régional des Mille-Îles (ACFOMI), en collaboration avec le
Réseau des services de santé en français de l'Est de l'Ontario (RSSFEO) et le Réseau local d'intégration des services
de santé du Sud-Est (RLISS SE), vous propose ce répertoire regroupant les ressources professionnelles de la santé
qui offrent des services en français.
Les services de l’ACFOMI s'adressent aux quelque 16 615 Francophones de la région qui s'étend de Trenton à
l'ouest, jusqu'à Brockville à l'est.
ACFOMI, in collaboration with the French Language Health Services Network of Eastern Ontario and the South East
Local Health Integration Network (LHIN) presents this directory that groups a wide variety of professional health
care providers that offer services in French.
ACFOMI caters to some 16,615 Francophones in the Thousand Island region that extends from Trenton in the West
to Brockville in the East.

Les centres d’accès aux soins communautaires (CASC) offrent un point d’accès unique aux services de santé et de
soutien personnel pour aider les gens à vivre de façon autonome chez eux, aider les enfants ayant des besoins en
matière de santé à fréquenter l’école, aider les aînés à effectuer la transition vers un foyer de soins de longue
durée ou un autre milieu de soins en résidence et aider les gens à trouver un médecin de famille.
Toute personne peut demander des services d’un centre d’accès aux soins communautaires.
Si vous, un membre de votre famille ou une connaissance pourriez bénéficier de nos services de santé et de
soutien, ou encore de nos services d’information et de recommandation, appelez-nous au 310 CASC (2272).
Nous sommes là tous les jours, de 8 h à 20 h.
